Temodar dosage reality check

This form should only be used to check that the dosage prescribed to you is in the correct range as specified on the package insert.

The doctor may have a reason for prescribing a different dosage, or a different schedule.

If this shows that you have the wrong dose, just ask your doctor why he prescribed the dose he did - to make sure there wasn't a math error.
